Smoking in youth: Isfahan Healthy Heart Project (IHHP)

Abstract
Given the lowering age of onset of smoking and increased prevalence of smoking in societies, studies are warranted to evaluate the knowledge, attitude and practice (KAP) of youngsters about smoking. This is a cross-sectional study involving youngster aged between 19 and 25 years in the provincial cities of Isfahan and Najaf-abad. A total of 1315 individuals were studied (49% male, 51% female).
Randomized multistage Sampling was used. A questionnaire was completed by trained health workers. The questionnaire collected demographic information and questions about KAP (Knowledge, Attitude and Practice) with regard to smoking. Data were statistically analyzed using chi-square, logistic regression and independent T-test.
Mean scores of knowledge and attitude in subjects were 8.34+/-1.97 and 15.39+/-2.43, respectively. There was no significant difference between men and women with respect to knowledge, however, a significant difference was observed with regard to attitude (P<0.0001). The difference was significant in both areas in subjects with high school education and beyond (P<0.0001). No difference was seen between rural and urban societies. 11.8% of youngsters smoked (22.5% men, 6.0% women) and cigarettes were the most commonly used products (88.9%). An average 10 cigarettes were used per day. The age of onset of smoking was 17.3±1.6 years in young men, and 20±2.9 years in young women. Sex, the smokers' parents, and education were related to the youngsters’ smoking habit.
The results of this study show that youngsters enjoyed good knowledge of smoking, but practice was not good in men. We recommend that training programs be designed and implemented, even in primary schools in order to prevent smoking in the first place and to stop it, should it begin.
